The Waymo AV was traveling southbound on [XXX], in the leftmost singular lane adjacent to [XXX], when an SUV exited a parking garage on the east side of the roadway and began to turn left onto [XXX], entering the Waymo AV's lane of travel. The Waymo AV braked and maneuvered to the right as the SUV continued to turn left, and the front right side of the SUV made contact with the rear left side of the Waymo AV. An occupied Zoox autonomous vehicle was traveling west on [XXX] toward [XXX] in Austin when it braked to a stop for a pedestrian and dog on a leash entering the crosswalk. A passenger car (Vehicle 2) traveling directly behind failed to slow down and rear ended the stopped Zoox vehicle. Shortly after, the Vehicle 2 driver reversed, inspected both vehicles, then returned to their vehicle and remained at the scene. The Zoox vehicle passenger reported feeling "dazed" and "kind of woozy" and requested emergency services. The police were notified and arrived at the scene. The Zoox fleet response team also arrived, and information was exchanged with the other party. Just as the Zoox vehicle started moving, an audible noise came from its fender, so it pulled over and was towed for further inspection. The vehicle was found to have sustained moderate damage to its rear end.

While the Waymo AV was stopped, a passenger car completing a left turn from southbound [XXX] traveling eastbound on [XXX] attempted to pass an SUV stopped behind the Waymo AV, and the right side of the passenger car made contact with the left side of the stopped SUV. The passenger car then continued forward and to the right and approached the Waymo AV from behind, and the front right side of the passenger car made contact with the rear left side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. All three vehicles sustained damage. The passengers in the Waymo AV were not belted at the time of the collision as they were preparing to exit the vehicle. At 5:59 AM CDT on July [XXX], 2026 in Parker County, Texas, an Aurora vehicle was traveling on a frontage road at approximately 27 mph at the intersection of [XXX] and [XXX] during a development mission. The Aurora vehicle was in manual mode. The Aurora vehicle was traveling straight through an intersection. The other vehicle was entering the intersection on the left side of the Aurora truck. The collision occurred when the Aurora truck was proceeding into the intersection with a light that had been green for several seconds and the other vehicle was proceeding against a red light. Following the collision, the vehicle operator brought the vehicle to a stop. The Aurora vehicle was towed from the location. The Aurora vehicle had previously been engaged in autonomy and the vehicle operator performed a disengagement well before the intersection and approximately 12 seconds before the collision. No injuries were reported. The Aurora vehicle sustained damage. Aurora will supplement or modify its reporting should additional relevant information become available.

On June [XXX], 2026 at 8:14 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was traveling southbound on [XXX] when it came to a stop in-lane to yield to an SUV. The SUV was facing south and rolling backwards northbound on [XXX], partially in the southbound lane and partially in the northbound lane. The Waymo AV began to reverse in lane. The SUV continued to roll backwards and the rear right side of the SUV made contact with the front left side of the Waymo AV. Both the Waymo AV and the SUV remained stopped momentarily, then the SUV continued to roll backwards, making further contact with the left side of the stationary Waymo AV, before briefly stopping. The SUV began to roll backwards again, at which point the driver of the SUV exited the moving SUV. The SUV continued to roll backwards northbound on [XXX], and the rear right side of the SUV made contact with the front left side of a second stationary Waymo AV ([XXX]) that was facing south, yielded in-lane behind the initial collision with the first Waymo AV. At the time of the impacts, both Waymo AVs' Level 4 ADS were engaged in autonomous mode. All three vehicles sustained damage, and both Waymo AVs were towed away. On June [XXX], 2026 at 7:34 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a carport in a parking lot at [XXX]. The Waymo AV was traveling eastbound, with its hazard lights activated in preparation for a passenger dropoff, in a parking lot at [XXX]. The Waymo AV maneuvered to the right and began to travel underneath a carport on the south side of the parking lot at a low rate of speed. As the Waymo AV proceeded forward, the sensor mounted on the roof of the Waymo AV made contact with a support beam on the underside of the carport. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. A passenger in the Waymo AV was not belted at the time of the collision as they were preparing to exit the vehicle. On June [XXX], 2026 at 11:41 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a passenger car on [XXX] at [XXX]. The Waymo AV was traveling southwestbound on [XXX] in autonomous mode as it approached the T-intersection with [XXX]. A passenger car initially stopped facing northwest on [XXX] proceeded forward and to the left, entering the intersection into the Waymo AV's lane of travel. A Zoox vehicle, operating in autonomous mode, was heading south on [XXX] in Las Vegas, slowing down for a red traffic signal at the [XXX] intersection, when a trailing passenger car (Vehicle 2) rear-ended it. The Zoox vehicle operator subsequently disengaged from autonomy. The collision resulted in moderate rear-end damage to the Zoox vehicle and major frontal damage to Vehicle 2, including frontal airbag deployment. Both vehicles were towed from the scene. Police responded to the scene, and no injuries were reported.

On June [XXX], 2026 at 10:58 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was traveling westbound in the left lane on [XXX] when an SUV traveling westbound in the right lane on [XXX], behind and to the right of the Waymo AV, began to shift to the left and enter the Waymo AV's lane of travel. As the SUV proceeded to pass the Waymo AV to the right and further entered the Waymo AV's lane of travel, the SUV began to brake, and the rear left side of the SUV made contact with the front right side of the Waymo AV. On June [XXX], 2026 at 3:12 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Chandler, Arizona was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was traveling southbound in the second from the right lane on [XXX] when an SUV traveling northbound in the opposing second from the right lane on [XXX] changed lanes and entered the adjacent center two-way left turn lane. The SUV continued maneuvering to the left, crossing the yellow lane divider and entering the Waymo AV's lane of travel. The Waymo AV braked and maneuvered to the right as the SUV proceeded forward. The front left side of the SUV made contact with the rear left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. On June [XXX], 2026 at 3:37 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving a van on [XXX] near [XXX]. The Waymo AV was stopped facing northeast in the rightmost of two designated right turn only lanes on [XXX] at a red light at the intersection with [XXX]. While the Waymo AV was stopped, a passenger exited a van initially stopped facing north in the marked crosswalk across [XXX], in front of the Waymo AV. The van subsequently began to reverse, and the rear left side of the van made contact with the front of a passenger car stopped in the adjacent leftmost designated right turn only lane as the passenger car began to reverse. The van and the passenger car both continued to reverse, and the rear right side of the van then made contact with the front left side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV and the passenger car sustained damage. On June [XXX], 2026 at 2:13 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX at the intersection of [XXX] and [XXX]. The AVRIDE AV was traveling southbound in autonomous mode on [XXX] as it approached the intersection with [XXX], where the traffic signal was flashing red. The AVRIDE AV came to a stop in compliance with the traffic control device. A pickup truck traveling behind the AVRIDE AV failed to stop and struck the rear of the AVRIDE AV. This maneuver resulted in the front of the pickup truck making contact with the rear of the AVRIDE AV. Both vehicles sustained minor damage. No injuries were reported.

On June [XXX], 2026 at 3:16 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Austin, TX on [XXX] near [XXX]. The AVRIDE AV was traveling northbound on [XXX] in autonomous mode in the far right lane, and had activated its turning signal in order to turn right onto [XXX]. Prior to the interesection with [XXX], an SUV was stopped perpendicular to and to the right of the AVRIDE AV waiting for traffic to clear in order to enter [XXX] from a parking lot. The SUV abruptly entered [XXX] and struck the right side of the AVRIDE AV. The safety driver attempted to avoid the conflict but was unable to. Both vehicles sustained damage. No injuries were reported.

On June [XXX], 2026, a minor property damage only incident occurred at approximately 10:00 PM Central Time, while a Gatik truck was operating with its Level 4 ADS system engaged in autonomous mode while travelling northbound on [XXX] just south of its intersection with [XXX] in Lowell, Arkansas. At the time of the incident, the Gatik AV was lawfully within the middle thru lane of a 3-lane roadway, and a 3rd party tractor trailer was positioned in the left turn lane biased toward the right edge of the lane. While the Gatik AV was passing the tractor trailer, the passenger side mirror of the tractor trailer brushed against the driverâs side mirror of the Gatik AV. The event resulted solely in minor, cosmetic property damage to the outer mirror housings. No structural body damage occurred, and no injuries were reported. Gatik is reporting this event under Request No. 2 of Standing General Order 2021-01 and may update or correct its reporting with additional information as or if it becomes available.

On June [XXX], 2026 at 10:34 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a heavy truck on [XXX] near [XXX]. The Waymo AV was traveling eastbound on [XXX] and approaching the intersection with [XXX]. A heavy truck was stopped facing northwestbound in the intersection, across eastbound [XXX], after turning left from northbound [XXX]. As the Waymo AV approached the stopped heavy truck, the Waymo AV maneuvered to the right and began slowing, and the heavy truck proceeded forward to complete the left turn onto [XXX]. The Waymo AV came to a stop near the south curb of [XXX] as the heavy truck began passing the Waymo AV to the left, and the left side of the heavy truck made contact with the rear left side of the stationary Waymo AV. Summary: The ADS was stopped at an intersection on a four-lane street in Lane #1, at a red left arrow light in the left-turn-only lane. An SUV behind the ADS activated their right turn signal and initiated a lane change into Lane #2, a straight-through lane, and collided with the left side of a passenger vehicle that was proceeding straight in Lane #2, before colliding with the right side of the ADS. There were no passengers.

On June [XXX], 2026 at 10:48 AM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Tempe, Arizona was in a collision involving a pickup truck on [XXX] near [XXX]. The Waymo AV was traveling northbound in the second from the right lane on [XXX] when it slowed to a stop for a queue of traffic at a red light at the intersection with [XXX]. As traffic ahead proceeded in the northbound lanes, the Waymo AV began to proceed at a low rate of speed, activated its left turn signal, and began to cross the white lane line into the right of two designated left turn lanes in a space between a pickup truck and a passenger car. The left turn lanes were stopped for a red light. When the light turned green, the Waymo AV and the pickup truck began proceeding forward, and the front right side of the pickup truck made contact with the left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage.
