On November [XXX], 2025 at 9:20 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a passenger car at [XXX], Terminal [XXX] at the [XXX]. The Waymo AV was traveling westbound in the right lane on [XXX], Terminal [XXX] when it slowed to a stop in a queue of traffic. A passenger car that was traveling westbound in the right lane of [XXX] slowed to a near stop directly behind the Waymo AV. While the Waymo AV was stopped and the passenger car slowly proceeded a second passenger car approached the first passenger car from behind, and the front of the second passenger car made contact with the rear of the first passenger car, and the front of the first passenger car then made contact with the rear of the stationary Waymo AV. On November[XXX], 2025 at 6:22 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving an SUV on [XXX] at [XXX]. The Waymo AV was traveling eastbound in the second from the right lane on [XXX] when an SUV passed the Waymo AV on the left, changed lanes in front of the Waymo AV into the Waymo AV's lane of travel, and came to a stop directly in front of the Waymo AV at the intersection with [XXX] at a green light. The Waymo AV came to a stop to yield to the stopped SUV, and the SUV then reversed and the rear of the SUV made contact with the front of the stationary Waymo AV. The SUV then proceeded forward momentarily, came to a stop, and reversed again into the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. On November [XXX], 2025 at 12:37 AM CT a Waymo Autonomous Vehicle ("Waymo AV") operating in Austin, Texas was in a collision involving a passenger car on [XXX] at [XXX]. The Waymo AV was stopped facing southwest in the designated left turn lane on [XXX] at the intersection with [XXX] with hazard lights activated. A passenger car traveling northeastbound on [XXX], in the Waymo AV's lane of travel and against the proper direction of traffic, approached the front of the Waymo AV, and the front left side of the passenger car made contact with the front left side of the stationary Waymo AV. Avride car was traveling on the side road next to an off ramp with a vehicle attempting to lane change into our lane. The other driver had the blinker ON indicating to lane change. Avride car drove past it leading the other vehicle to have to wait to lane change. As Avride car approaches the intersection at a red light, the other vehicle then comes out from behind the rover to the left lane again, cuts the rover off and then drives off.

An SUV that was initially stopped directly behind the Waymo AV proceeded forward and came to a stop directly behind the stationary Waymo AV. While the Waymo AV and the SUV were stopped, a pickup truck that was initially stopped directly behind the SUV approached the stopped SUV from behind and the front of the pickup truck made contact with the rear of the stationary SUV. The front of the SUV then made contact with the rear of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. A person on a motor scooter (VRU) approached the ADS from behind and moved into a gap directly behind the ADS. As the VRU was directly behind the stopped ADS, the VRU steered right and made contact with the ADS's rear end. Afterwards the VRU hopped the right curb and continued to proceed straight on the sidewalk. Safety monitor and passenger were present in the vehicle.

The Waymo AV was traveling westbound in the second from the right lane on [XXX], proceeding straight at a green light through the intersection with [XXX]. As the Waymo AV approached the exit of the intersection, a passenger car traveling eastbound on [XXX] in the leftmost designated left turn lane proceeded to make an unprotected left turn against a red light onto [XXX], entered the Waymo AV's lane of travel, the Waymo AV braked and maneuvered to the right, and the front of the passenger car made contact with the left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. While the Waymo AV was stopped, a passenger car traveling southbound on [XXX] approached the Waymo AV from behind at a high rate of speed and began to maneuver to the left, and the front right side of the passenger car made contact with the rear left side of the stationary Waymo AV. The front of the passenger car then made contact with the rear of a second passenger car that was stopped at the stop sign in front of and to the left of the Waymo AV. The driver of the second passenger car was transported to a hospital by an ambulance with unknown injuries. During normal autonomous operation, the May autonomous shuttle (Ego) was traveling through a narrow section of roadway in a parking lot bordered by parked vehicles on the left and a retaining wall on the right on experimental software version v29.0.20. As the Ego proceeded, the system initially recognized the static retaining wall as a large, non-rectangular object. As the Ego approaches the wall, the resulting density of lidar returns on the object (wall) exceeds the detectors maximum limitation. The newer lidar points, toward the forward edge of the elongated object, exceeding the limit were filtered out, causing the track to disappear when the ego approached 5m from the track (i.e. the wall). The situation was exacerbated by the fact that the road edge was incorrectly marked in that section. The road edge was drawn through the retaining wall and thus the vehicle did not have any prior knowledge about the edge of the road. As a result, the Ego drove too close to the wall, scraping it at a speed of 2 meters per second while driving straight (i.e. in lane-nominal policy) with no nudge or veer corrections. There was no damage to the wall. The limitation leading to the lost track affects Software versions v26.0 (when the Open World Tracklet v2 feature was introduced) through v31.0. This is the only known instance of this failure mode, and in v32.0 an optimization was made to remove the LIDAR threshold entirely and mitigate the vulnerability. Furthermore, the incorrect road edge annotation was corrected shortly after this incident and the map updated for the deployed vehicles.

On November [XXX], 2025 at 5:59 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a passenger car on [XXX] near [XXX]. The Waymo AV was traveling southbound on [XXX] in the left lane. A passenger car traveling eastbound on [XXX] proceeded to make a right turn onto [XXX] and crossed into the Waymo AV's lane of travel, and the front left side of the passenger car made contact with the rear right side of the Waymo AV. On November [XXX], 2025 at 12:05 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a passenger car on [XXX] near [XXX]. On November [XXX], 2025 at 9:31 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving an SUV in a parking lot at [XXX]. The Waymo AV was stopped facing east at the curb in a parking lot at [XXX] with its hazard lights activated for a passenger drop-off. While the Waymo AV was stopped, an SUV traveling westbound in the parking lot initiated a left turn into a passenger loading zone driveway and slowed to a stop facing south, within the Waymo AV's lane of travel. The Waymo AV proceeded forward and maneuvered to the left to pass the rear of the SUV. While the Waymo AV was passing the SUV, the SUV began to reverse and the rear driver side of the SUV made contact with the rear passenger side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage.
