On July [XXX], 2026 at 5:40 PM MST a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was involved in a collision with an SUV on [XXX] near [XXX]. The Waymo AV was traveling southbound on [XXX], in the leftmost singular lane adjacent to [XXX], when an SUV exited a parking garage on the east side of the roadway and began to turn left onto [XXX], entering the Waymo AV's lane of travel. The Waymo AV braked and maneuvered to the right as the SUV continued to turn left, and the front right side of the SUV made contact with the rear left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. An occupied Zoox autonomous vehicle was traveling west on [XXX] toward [XXX] in Austin when it braked to a stop for a pedestrian and dog on a leash entering the crosswalk. A passenger car (Vehicle 2) traveling directly behind failed to slow down and rear ended the stopped Zoox vehicle. Shortly after, the Vehicle 2 driver reversed, inspected both vehicles, then returned to their vehicle and remained at the scene. The Zoox vehicle passenger reported feeling "dazed" and "kind of woozy" and requested emergency services. The police were notified and arrived at the scene. The Zoox fleet response team also arrived, and information was exchanged with the other party. Just as the Zoox vehicle started moving, an audible noise came from its fender, so it pulled over and was towed for further inspection. The vehicle was found to have sustained moderate damage to its rear end.

At 5:59 AM CDT on July [XXX], 2026 in Parker County, Texas, an Aurora vehicle was traveling on a frontage road at approximately 27 mph at the intersection of [XXX] and [XXX] during a development mission. The Aurora vehicle was in manual mode. The Aurora vehicle was traveling straight through an intersection. The other vehicle was entering the intersection on the left side of the Aurora truck. The collision occurred when the Aurora truck was proceeding into the intersection with a light that had been green for several seconds and the other vehicle was proceeding against a red light. Following the collision, the vehicle operator brought the vehicle to a stop. The Aurora vehicle was towed from the location. The Aurora vehicle had previously been engaged in autonomy and the vehicle operator performed a disengagement well before the intersection and approximately 12 seconds before the collision. Reported narrative: It was reported that during a da Vinci-assisted sliding hiatal hernia procedure, after sealing and cutting of an unspecified branch of the aorta, bleeding occurred and the patient ultimately expired.The surgeon reported that the Vessel Sealer Curved (VSC) instrument seal and cut cycle worked as expected with no error messages. However, when the jaws of the VSC were opened, the sealed branch started to hemorrhage. The procedure continued robotically for approximately one hour in attempts to fully stop the bleeding which was able to be controlled at multiple points after the initial seal. The estimated blood loss during the robotic portion was approximately 2 liters when a mass transfusion protocol was initiated and the procedure was converted to a thoracotomy but the surgeon was unable to control the bleeding. The aorta was then cross-clamped and the bleeding stopped; however, the patient expired in the operating room. The surgeon reports there were no error messages or robotic-related complications that occurred during the procedure. Specifically, the VSC worked as expected with no error messages or complications with sealing during use.

On June [XXX], 2026 at 1:29 PM ET a Waymo Autonomous Vehicle ("Waymo AV") operating in Miami, Florida was in a collision involving a pickup truck on [XXX] near [XXX]. The Waymo AV was traveling westbound in the designated left turn lane on [XXX] with its left turn signal activated in preparation for a left turn at the green light at the intersection with [XXX]. As the Waymo AV completed the left turn and continued traveling northbound in the leftmost lane on [XXX], a pickup truck traveling northbound on [XXX] in the second from the left lane, adjacent and to the right of the Waymo AV, maneuvered left entering the Waymo AV's lane of travel, and the front left side of the pickup truck made contact with the right side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. On June [XXX], 2026 at 12:13 AM PT a Waymo Autonomous Vehicle ("Waymo AV" operating in San Francisco, California was in a collision involving a passenger car on [XXX] near [XXX]. The Waymo AV was parked facing southwest in the rightmost lane on [XXX], with its hazard lights activated for a passenger pickup, behind a queue of traffic stopped in the rightmost lane on [XXX] for a red light at the intersection with [XXX]. While the Waymo AV was parked, a passenger car that was initially stopped in front of the Waymo AV reversed, and the rear of the passenger car made contact with the front of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. A Zoox vehicle, operating in autonomous mode, was heading south on [XXX] in Las Vegas, slowing down for a red traffic signal at the [XXX] intersection, when a trailing passenger car (Vehicle 2) rear-ended it. The Zoox vehicle operator subsequently disengaged from autonomy. The collision resulted in moderate rear-end damage to the Zoox vehicle and major frontal damage to Vehicle 2, including frontal airbag deployment. Both vehicles were towed from the scene. Police responded to the scene, and no injuries were reported.

On June [XXX], 2026 at 2:13 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX at the intersection of [XXX] and [XXX]. The AVRIDE AV was traveling southbound in autonomous mode on [XXX] as it approached the intersection with [XXX], where the traffic signal was flashing red. The AVRIDE AV came to a stop in compliance with the traffic control device. A pickup truck traveling behind the AVRIDE AV failed to stop and struck the rear of the AVRIDE AV. This maneuver resulted in the front of the pickup truck making contact with the rear of the AVRIDE AV. Both vehicles sustained minor damage. No injuries were reported.

On June [XXX], 2026 at 3:16 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Austin, TX on [XXX] near [XXX]. The AVRIDE AV was traveling northbound on [XXX] in autonomous mode in the far right lane, and had activated its turning signal in order to turn right onto [XXX]. Prior to the interesection with [XXX], an SUV was stopped perpendicular to and to the right of the AVRIDE AV waiting for traffic to clear in order to enter [XXX] from a parking lot. The SUV abruptly entered [XXX] and struck the right side of the AVRIDE AV. The safety driver attempted to avoid the conflict but was unable to. Both vehicles sustained damage. No injuries were reported.

On June [XXX], 2026, a minor property damage only incident occurred at approximately 10:00 PM Central Time, while a Gatik truck was operating with its Level 4 ADS system engaged in autonomous mode while travelling northbound on [XXX] just south of its intersection with [XXX] in Lowell, Arkansas. At the time of the incident, the Gatik AV was lawfully within the middle thru lane of a 3-lane roadway, and a 3rd party tractor trailer was positioned in the left turn lane biased toward the right edge of the lane. While the Gatik AV was passing the tractor trailer, the passenger side mirror of the tractor trailer brushed against the driverâs side mirror of the Gatik AV. The event resulted solely in minor, cosmetic property damage to the outer mirror housings. No structural body damage occurred, and no injuries were reported. Gatik is reporting this event under Request No. 2 of Standing General Order 2021-01 and may update or correct its reporting with additional information as or if it becomes available.

On June [XXX], 2026 at 12:49 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a heavy truck on [XXX] at [XXX]. The Waymo AV was stopped facing north in the left lane on [XXX] for a red light at the intersection with [XXX]. After the light turned green the Waymo AV proceeded straight, with its right turn signal activated in preparation for a lane change to the adjacent right lane and maneuvered to the right as it began to enter the intersection. As the Waymo AV maneuvered further towards the right and entered the intersection, the Waymo AV began to slow to yield to pedestrians crossing [XXX] against the traffic light in the marked crosswalk. A heavy truck initially stopped in the right lane began to proceed forward and approached the Waymo AV from behind, and the front left side of the heavy truck made contact with the rear right side of the Waymo AV. At the time of impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. Waymo is reporting this crash under Request No. 2 of Standing General Order 2021-01. Waymo may supplement or correct its reporting with additional information as it may become available.

An occupied Zoox autonomous vehicle was traveling north on [XXX], approaching the intersection of [XXX] in Las Vegas, when an active emergency vehicle, with lights and sirens, was westbound on [XXX] and rapidly approaching the intersection. The Zoox vehicle appropriately braked hard to a stop to yield to the approaching active emergency vehicle. A pickup truck (Vehicle 2), traveling behind the Zoox vehicle, failed to stop and rear ended the stopped Zoox vehicle. The police were notified and arrived at the scene. Zoox vehicle's camera footage shows that the driver of Vehicle 2 was holding a device in their right hand leading up to the collision. The driver of Vehicle 2 was cited for failing to decrease speed. The Zoox vehicle was towed from the scene and found to have sustained moderate damage. The passengers initially declined medical attention; however, after the Zoox fleet response team arrived, one passenger requested transport to a hospital.

On June [XXX], 2026 at 11:37 PM ET a Waymo Autonomous Vehicle ("Waymo AV") operating in Atlanta, Georgia was in a collision involving a passenger car on [XXX] near [XXX]. The Waymo AV was traveling eastbound in the second from right lane on [XXX] when it began slowing as it approached a queue of traffic stopped for a red light at the intersection with [XXX]. While the Waymo AV was slowing, a passenger car traveling eastbound on [XXX] in the second from right lane approached the Waymo AV from behind, and the front of the passenger car made contact with the rear of the Waymo AV. At the time of impact , the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Waymo is reporting this crash under Request No. 2 of Standing General Order 2021-01. Waymo may supplement or correct its reporting with additional information as it may become available.
