On June [XXX], 2026 at 2:54 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling eastbound on [XXX] in autonomous mode when it approached a passenger car parked on the left side of the roadway. As the AVRIDE AV approached the parked vehicle, the driver-side door of the passenger car was opened into the roadway. There was insufficient room to open the door safely without interfering with traffic. The AVRIDE AV continued traveling straight ahead, resulting in light contact between its left side mirror and the driver-side door of the passenger car. Neither vehicle sustained damage. No injuries were reported.

After the light turned green, the Waymo AV proceeded forward and the heavy truck initiated a wide right turn onto [XXX], maneuvering to the left and crossing further into the leftmost lane, further entering the Waymo AV's lane of travel. On June {xxx}, 2026 at 10:06 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX in a parking lot near [XXX]. The AVRIDE AV was traveling northeastbound through the parking lot in autonomous mode as it approached a four-way stop-controlled intersection. The AVRIDE AV came to a complete stop and then proceeded straight having established the right of way. An SUV to its left failed to stop, proceeded into the intersection making a left turn and struck the AVRIDE AV. This maneuver resulted in the right side of the SUV making contact with the front left of the AVRIDE AV. Both vehicles sustained damage. No injuries were reported.

On June [XXX], 2026 at 8:27 PM CT a Waymo Autonomous Vehicle ("Waymo AV") operating in Austin, Texas was in a collision involving a passenger car on [XXX] near [XXX]. The Waymo AV was stopped facing southwest in the rightmost lane on [XXX] for a passenger car stopped directly in front of the Waymo AV with its hazard lights activated. The Waymo AV began to reverse at a low rate of speed, yielding to an SUV exiting a parking lot on the west side of [XXX] behind the Waymo AV. While the Waymo AV was reversing, the passenger car began to reverse, and the rear of the passenger car made contact with the front of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On June [XXX], 2026 at 7:40 AM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX at the intersection of [XXX] and [XXX]. The AVRIDE AV was traveling southeastbound on [XXX] in autonomous mode in rainy conditions as it approached the intersection with [XXX] and began slowing to make a right turn. As the AVRIDE AV decelerated, a pickup truck traveling behind it failed to stop and struck the rear of the AVRIDE AV. The impact caused the pickup truck to be pushed rearward into the path of an SUV traveling behind it, resulting in contact between the rear of the pickup truck and the front of the SUV. All three vehicles sustained damage. No injuries were reported.

An unoccupied Zoox autonomous vehicle was making a left turn from eastbound [XXX] onto northbound [XXX] in Las Vegas when an active emergency vehicle, with lights and sirens, was rapidly approaching the intersection and conducting a right turn onto northbound [XXX]. The Zoox vehicle was slowing to yield to the approaching active emergency vehicle, when an SUV (Vehicle 2) traveling behind it failed to stop, and rear ended the slowing Zoox vehicle. Two other passenger vehicles (Vehicles 3 & 4) were following behind Vehicle 2. Vehicle 4 appeared to have made contact with Vehicle 3 from behind. Neither Vehicle 3 nor Vehicle 4 contacted the Zoox vehicle, and it does not appear that either contacted Vehicle 2. The driver of Vehicle 2 then pulled over ahead and contacted Zoox support to exchange information. The police were notified, but did not arrive at the scene. The Zoox vehicle was towed from the scene and found to have sustained moderate damage.

A Zoox vehicle, operating in autonomous mode, was stopped in the westbound left-turn lane of [XXX], yielding to oncoming traffic to turn left onto [XXX] in Austin under a flashing yellow arrow. Eastbound traffic on [XXX] was backed up and slowing down. After receiving a hand gesture from the driver of a slowing SUV in the oncoming lane, the Zoox vehicle operator disengaged from autonomy and proceeded with the left turn. At that time, a pickup truck (Vehicle 2) traveling in the right lane of eastbound [XXX] quickly approached the intersection. Although the driver of Vehicle 2 attempted to steer left, the vehicle collided with the rear right area of the Zoox vehicle in the intersection. Vehicle 2's left side subsequently made contact with the front right corner of the oncoming SUV. Both the Zoox vehicle and Vehicle 2 sustained moderate damage, and the Zoox vehicle was towed. No injuries were reported, and police were notified but did not respond to the scene.

On June [XXX], 2026 at 4:07 AM a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving a heavy truck on [XXX] at [XXX]. The Waymo AV was traveling northeastbound in the third from the right lane approaching the intersection with [XXX]. A heavy truck was traveling in the second from the right lane, ahead and to the right of the Waymo AV. As the Waymo AV was entering the intersection, the heavy truck began to turn left onto [XXX], from the second from the right lane, entering the Waymo AV's lane of traffic. The Waymo AV began to maneuver to the left, and the front left of the heavy truck tractor made contact with the right side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. On June [XXX], 2026 at 1:45 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving a pickup truck on [XXX] at [XXX]. The Waymo AV was stopped facing southwest in the leftmost designated left turn lane on [XXX] with its left turn signal activated in preparation for an unprotected left turn at the red light at the intersection with [XXX]. After the light turned green, the Waymo AV proceeded forward momentarily, then slowed to stop before entering the intersection yielding to northeastbound traffic. While the Waymo AV was stopped, a pickup truck initially stopped facing southwest on [XXX] in the adjacent leftmost throughlane, behind and to the right of the Waymo AV, proceeded forward to pass the Waymo AV on the right and crossed the solid white lane line into the Waymo AV's lane of travel. The Waymo AV initiated the left turn onto [XXX] as the pickup truck proceeded forward, and the left side of the pickup truck made contact with the right side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On May [XXX], 2026 at 2:10 PM, an AVRIDE autonomous vehicle(the "AVRIDE AV") was in a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling northwestbound in autonomous mode on [XXX] when it approached a red light at a railroad crossing. After stopping at the red light and waiting for the railroad crossing gate to raise, the AVRIDE AV began to drive forward. As the AVRIDE AV drove forward, the railroad crossing gate began to lower again. As the railroad crossing gate lowered, the AVRIDE AV continued to drive forward. This maneuver resulted in the top of the AVRIDE AV making contact with the railroad crossing gate. Neither the AVRIDE AV nor railroad crossing gate sustained damage. No injuries were reported.
