On June [XXX], 2026 at 2:23 PM, an AVRIDE autonomous vehicle(the "AVRIDE AV") was in a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling southbound on [XXX] in autonomous mode in the middle lane of the one-way street. An SUV was traveling southbound slightly ahead of the AVRIDE AV in the far right lane. Traffic ahead of the SUV was stopped, and without signaling the SUV made an unsafe lane change into the AVRIDE AV's lane of travel. This maneuver by the SUV resulted in the left side of the SUV making contact with the right side of the AVRIDE AV. Both vehicles sustained damage. No injuries were reported.

An unoccupied Zoox autonomous vehicle was making a left turn from eastbound [XXX] onto northbound [XXX] in Las Vegas when an active emergency vehicle, with lights and sirens, was rapidly approaching the intersection and conducting a right turn onto northbound [XXX]. The Zoox vehicle was slowing to yield to the approaching active emergency vehicle, when an SUV (Vehicle 2) traveling behind it failed to stop, and rear ended the slowing Zoox vehicle. Two other passenger vehicles (Vehicles 3 & 4) were following behind Vehicle 2. Vehicle 4 appeared to have made contact with Vehicle 3 from behind. Neither Vehicle 3 nor Vehicle 4 contacted the Zoox vehicle, and it does not appear that either contacted Vehicle 2. The driver of Vehicle 2 then pulled over ahead and contacted Zoox support to exchange information. The police were notified, but did not arrive at the scene. The Zoox vehicle was towed from the scene and found to have sustained moderate damage.

On June [XXX], 2026 at 10:35 AM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX at the intersection of [XXX] and [XXX]. The AVRIDE AV was traveling northbound on [XXX] in autonomous mode in the far left lane as it approached the intersection with [XXX]. [XXX] is a one-way street, and the AVRIDE AV had the right-of-way while traffic on [XXX] was controlled by a stop sign. An SUV failed to stop and made a right turn onto oncoming traffic on [XXX], traveling in the wrong direction on the one-way roadway. This maneuver resulted in the front of the SUV making contact with the left front of the AVRIDE AV. Both vehicles sustained damage. No injuries were reported

A Zoox vehicle, operating in autonomous mode, was stopped in the westbound left-turn lane of [XXX], yielding to oncoming traffic to turn left onto [XXX] in Austin under a flashing yellow arrow. Eastbound traffic on [XXX] was backed up and slowing down. After receiving a hand gesture from the driver of a slowing SUV in the oncoming lane, the Zoox vehicle operator disengaged from autonomy and proceeded with the left turn. At that time, a pickup truck (Vehicle 2) traveling in the right lane of eastbound [XXX] quickly approached the intersection. Although the driver of Vehicle 2 attempted to steer left, the vehicle collided with the rear right area of the Zoox vehicle in the intersection. Vehicle 2's left side subsequently made contact with the front right corner of the oncoming SUV. Both the Zoox vehicle and Vehicle 2 sustained moderate damage, and the Zoox vehicle was towed. No injuries were reported, and police were notified but did not respond to the scene.

On May [XXX], 2026 at 1:52 PM a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a pickup truck on [XXX] near [XXX]. The Waymo AV was traveling northbound in the third from the right lane on [XXX] with its left turn signal activated as it approached the intersection with [XXX]. As the Waymo AV was slowing to yield to a queue of traffic stopped for a red light at the intersection ahead, it initiated a lane change to the adjacent designated left turn lane, into a space between a stopped pickup truck and an SUV, and came to a stop partially in the third from the right lane, and partially in front of a pickup truck stopped in the adjacent designated left turn lane. As the light turned to a green arrow and traffic began to turn left, the Waymo AV and the pickup truck began to proceed forward, and the front right side of the pickup truck made contact with the left side of the Waymo AV. While the Waymo AV was stopped, a passenger car traveling westbound in the left lane on [XXX] approached the Waymo AV from behind and slowed, and a second passenger car traveling westbound in the left lane on [XXX] approached the slowing passenger car from behind, and the front of the second passenger car made contact with the rear of the first passenger car. As a result of this collision, the first passenger car was pushed forward, and the front of the passenger car made contact with the rear of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. On May [XXX], 2026 at 10:31 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in West Hollywood, California was in a collision involving an SUV on [XXX] near [XXX]. A Zoox vehicle operating in autonomous mode was gradually slowing for oncoming traffic while preparing to turn left onto westbound [XXX] from [XXX] in Miami, when a pickup truck (Vehicle 2) rear-ended it while executing a right turn from [XXX] onto [XXX]. On May [XXX], 2026 at 3:42 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was adjacent to a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling southbound in the center lane in autonomous mode on [XXX] when it stopped at a red traffic signal at the intersection of [XXX] and [XXX]. The AVRIDE AV was being followed by a passenger car. When the traffic signal turned green, the AVRIDE AV proceeded into the intersection and activated its left turn signal to initiate a lane change. The trailing passenger car also moved into the left lane and attempted to overtake the AVRIDE AV while the AVRIDE AV was moving left. Upon detecting the presence of the passenger car, the AVRIDE AV applied its brakes to avoid a collision. The passenger car also braked. A box truck traveling behind the passenger car was unable to stop in time and struck the rear of the passenger car. The passenger car and the box truck sustained damage. No injuries were reported.

The Waymo AV was stopped facing west in the leftmost lane on [XXX] to yield to an SUV stopped near the south curb, partially in the leftmost lane and partially in the passenger loading zone. After traffic in the adjacent right lane cleared, the Waymo AV proceeded to pass the SUV to the right and maneuver back into the leftmost lane. As the Waymo AV proceeded forward in the leftmost lane, a second SUV, initially stopped in the passenger loading zone, departed to the right and entered the Waymo AV's lane of travel. The Waymo AV maneuvered to the right as the second SUV continued forward, and the front right side of the second SUV made contact with the rear left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. Summary: The Tesla ADS was proceeding straight on a residential street behind a construction truck. As the construction truck came to a reduced roadway width created by cars parked on the side of the road, the ADS followed behind the construction truck through the area of reduced roadway width. The construction truck stopped abruptly, forcing the Tesla ADS to stop in the area of reduced roadway width, then the truck reversed into the Tesla ADS's front right. The ADS did not have any passenger present in the vehicle.
