On May [XXX], 2026 at 2:10 PM, an AVRIDE autonomous vehicle(the "AVRIDE AV") was in a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling northwestbound in autonomous mode on [XXX] when it approached a red light at a railroad crossing. After stopping at the red light and waiting for the railroad crossing gate to raise, the AVRIDE AV began to drive forward. As the AVRIDE AV drove forward, the railroad crossing gate began to lower again. As the railroad crossing gate lowered, the AVRIDE AV continued to drive forward. This maneuver resulted in the top of the AVRIDE AV making contact with the railroad crossing gate. Neither the AVRIDE AV nor railroad crossing gate sustained damage. No injuries were reported.

The Waymo AV remained stopped as the pickup truck passed the Waymo AV, and then a passenger car initially stopped behind the pickup truck began to pass the Waymo AV to the left, and the right side of the passenger car made contact with the left side of the stationary Waymo AV. The Waymo AV was stopped facing southeast on [XXX] in the right lane with its right turn signal activated in preparation to make a right turn onto southwestbound [XXX], yielding to a pedestrian crossing [XXX] from the east side of the curb within the marked crosswalk. As the pedestrian exited the crosswalk, the Waymo AV proceeded forward at a low rate of speed, and an SUV initially stopped directly behind the Waymo AV approached the Waymo AV from behind and the front of the SUV made contact with the rear of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. The Waymo AV was traveling southwestbound in the left lane on [XXX] and proceeding straight at the intersection with [XXX] when the Waymo AV slowed to a stop within the intersection to yield to bicyclists traveling southeastbound on [XXX] and crossing into the Waymo AV's lane of travel.While the Waymo AV was stopped and the bicyclists proceeded across the Waymo AV's lane of travel, a bus traveling northeastbound in the left lane on [XXX], with its left signal activated for a left turn onto [XXX], began to pass the Waymo AV to the left, maneuvered to the left momentarily, and then proceeded to continue straight on [XXX], and the rear left side of the bus made contact with the rear left side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. On May [XXX], 2026 at 3:42 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was adjacent to a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling southbound in the center lane in autonomous mode on [XXX] when it stopped at a red traffic signal at the intersection of [XXX] and [XXX]. The AVRIDE AV was being followed by a passenger car. When the traffic signal turned green, the AVRIDE AV proceeded into the intersection and activated its left turn signal to initiate a lane change. The trailing passenger car also moved into the left lane and attempted to overtake the AVRIDE AV while the AVRIDE AV was moving left. Upon detecting the presence of the passenger car, the AVRIDE AV applied its brakes to avoid a collision. The passenger car also braked. A box truck traveling behind the passenger car was unable to stop in time and struck the rear of the passenger car. The passenger car and the box truck sustained damage. No injuries were reported.

Summary: The Tesla ADS was proceeding straight on a residential street behind a construction truck. As the construction truck came to a reduced roadway width created by cars parked on the side of the road, the ADS followed behind the construction truck through the area of reduced roadway width. The construction truck stopped abruptly, forcing the Tesla ADS to stop in the area of reduced roadway width, then the truck reversed into the Tesla ADS's front right. The ADS did not have any passenger present in the vehicle.

The Waymo AV was stopped facing east on [XXX] at a stop sign at the intersection with [XXX] with its left turn signal activated at the intersection with [XXX], preparing to turn left onto [XXX]. While the Waymo AV remained stopped, yielding to northbound traffic on [XXX], a pickup truck initially stopped behind the Waymo AV proceeded forward, and the front of the pickup truck made contact with the rear of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On May [XXX], 2026 at 9:29 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Culver City, California was in a collision involving a passenger car on [XXX] at [XXX]. The Waymo AV was stopped facing northwest in the left lane on [XXX] in a queue of traffic for a red light at the intersection with [XXX]. After the light turned green and traffic ahead advanced, the Waymo AV proceeded forward and then began to slow before entering the intersection, as the vehicle ahead of the Waymo AV had stopped within the intersection. While the Waymo AV was slowing, a passenger car traveling northwestbound in the left lane on [XXX] approached the Waymo AV from behind, and the front of the passenger car made contact with the rear of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On May [XXX], 2026 at 2:29 AM CT a Waymo Autonomous Vehicle ("Waymo AV") operating in Austin, Texas was in a collision involving a pickup truck in a parking lot at [XXX]. The Waymo AV was traveling southeastbound in a parking lot at [XXX], when it slowed to a stop to yield to a pickup truck that was traveling northwestbound and to the left of the Waymo AV. While the Waymo AV was stopped, the pickup truck came to a stop adjacent to the Waymo AV, and the Waymo AV activated its hazard lights for a passenger dropoff. On May [XXX], 2026 at 3:59 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a pickup truck on [XXX] at [XXX]. The Waymo AV was traveling westbound in the second to the right lane on [XXX] and proceeding straight at a green light at the intersection with [XXX]. As the Waymo AV began to cross the intersection, a pickup truck traveling northbound in the designated right turn lane on [XXX]t proceeded to enter the intersection against a red light and began to maneuver to the left. The Waymo AV braked and maneuvered to the right as the pickup truck continued to proceed forward, and the front of the pickup truck made contact with the left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Two passengers in the Waymo AV were not belted at the time of the collision. Two passengers in the Waymo AV were transported to a hospital by an ambulance with moderate injuries. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because individuals involved were transported from the scene to a hospital for medical treatment and a vehicle involved was towed away. An unoccupied Zoox autonomous vehicle was traveling south on [XXX] on a green light at [XXX] in San Francisco when a pickup truck (Vehicle 2) executed an abrupt left turn from northbound [XXX] and accelerated into the Zoox vehicle's right-of-way. The Zoox vehicle braked hard, but contact was unavoidable. Vehicle 2's front bumper struck the Zoox vehicle's left side door and body panels. After reversing, Vehicle 2 cleared the intersection, pulled over temporarily, and then left without exchanging information. Shortly afterward, the Zoox vehicle cleared the intersection, pulled over, and was towed for inspection. It was found to have sustained moderate damage. No injuries were reported, and police were not called.

As the Waymo AV began to change lanes, a pickup truck that was initially stopped behind the Waymo AV began to change lanes into the leftmost lane, attempting to pass the Waymo AV on the left. While the pickup truck was changing lanes, the Waymo AV came to a stop, partially in the center lane and partially in the rightmost lane. The pickup truck continued with the lane change, and the right side of the pickup truck made contact with the rear left side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. On May [XXX], 2026 at 9:05 PM, an AVRIDE autonomous vehicle(the "AVRIDE AV") was in a collision in Austin, TX at the intersection of East [XXX] and South [XXX]. The AVRIDE AV was traveling westbound in autonomous mode on [XXX] as it approached a traffic light at the intersection with South [XXX]. The traffic light showed green as the AVRIDE AV approached the intersection and began to proceed straight through the green traffic light. As the AVRIDE AV began to enter the intersection, an SUV traveling southbound on [XXX] also entered the intersection from the right of the AVRIDE AV. While entering the intersection, the SUV went against traffic rules and drove through a red light. This maneuver resulted in the left side of the SUV making contact with the front of the AVRIDE AV. Both vehicles sustained damage. No injuries were reported.

On May [XXX], 2026 at 1:03 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Burlingame, California was in a collision involving an animal on [XXX] near [XXX]. The Waymo AV was traveling southeastbound on [XXX] when a deer ran into the roadway from the right side and entered the Waymo AV's lane of travel, and the front right side of the Waymo AV made contact with the deer. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. The passenger in the Waymo AV was not belted at the time of the collision. On May [XXX], 2026 at 12:02 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a bus on [XXX] near [XXX]. The Waymo AV was traveling northbound on [XXX] and slowing for a queue of traffic for a red light at the intersection with [XXX] when the Waymo AV passenger opened the rear right door, causing the Waymo AV to stop in-lane and activate its hazard lights for passenger dropoff. While the Waymo AV was stopped, a shuttle bus traveling southbound on [XXX] approached the Waymo AV from the front and slowed to a stop momentarily. The shuttle bus proceeded forward to pass the Waymo AV to the left, between the Waymo AV and a fire truck that was stopped facing north partially in the bike lane and in-lane on southbound [XXX], and the left side of the shuttle bus made contact with the rear left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. On May [XXX], 2026 at 8:46 AM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, TX on [XXX] near [XXX]. The AVRIDE AV was traveling northbound on [XXX] in autonomous mode as it approached a hotel parking lot exit located on the left side of the roadway. As the AVRIDE AV proceeded straight ahead in the left lane, a pickup truck exiting the parking lot began making a left turn onto [XXX] without stopping as required before entering the roadway. This maneuver resulted in the right front of the pickup truck making contact with the left front of the AVRIDE AV. Both vehicles sustained damage. No injuries were reported.

On May [XXX], 2026 at 2:43 PM, an AVRIDE autonomous vehicle(the "AVRIDE AV") was in a collision in Dallas, TX at the intersection of [XXX] and [XXX]. The AVRIDE AV was traveling northeastbound in the left lane on [XXX] in autonomous mode as it approached the green showing traffic light at the intersection with [XXX]. As the AVRIDE AV approached the intersection, an SUV traveling southeastbound on [XXX] also approached the intersection from the left of the AVRIDE AV. The SUV went against traffic rules and entered the intersection without stopping at the traffic light which was showing red. This maneuver by the SUV resulted in the front of the SUV making contact with the left side of the AVRIDE AV. Both vehicles sustained damage. No injuries were reported.

On May [XXX], 2026 at 4:11 pm CT, a low-hanging utility cable made contact with a May Mobility Autonomous Vehicle (AV) operating in Arlington, Texas on [XXX] near [XXX] The May Mobility AV was traveling Eastbound on [XXX]. As the May Mobility AV passes [XXX] a loop of an unsecured, low-hanging utility cable that was dangling above the right lane of travel made contact with the front right A-pillar near the windshield of the May Mobility AV. At the time of impact, the May Mobility AV was in autonomous mode, traveling at 33 MPH. The impact left scuffs on the May Mobility AV that were buffed out, not requiring repair. Damage to the utility cable beyond the original state of disrepair is unknown. May Mobility is reporting this crash under Request No. 2 of Standing General Order 2021-01. May Mobility may supplement or correct its reporting with additional information as it becomes available.
