AFTER ATTEMPTING TO POWER THE SYSTEM USING MULTIPLE OUTLETS THROUGHOUT THE HOSPITAL, WE WERE STILL UNABLE TO MAINTAIN POWER. THE PLUG INDICATOR LIGHT DOES NOT ILLUMINATE WHEN CONNECTED, AND THE BATTERY INDICATOR ONLY FLASHES. AS A RESULT, THE ROBOT CANNOT BE KEPT POWERED ON. The Waymo AV was traveling eastbound on [XXX] when it began to slow to yield to a slow-moving van traveling eastbound on the north side of [XXX] (against the proper direction of traffic) with its hazard lights activated. While the Waymo AV was slowing, the van changed lanes to the right, then maneuvered to the left and came to a stop momentarily, facing northeast at the north curb on [XXX]. The van then reversed, the Waymo AV slowed to a stop to yield to the van, and as the van reversed, the rear left side of the van made contact with the front left side of the stationary Waymo AV. On December [XXX], 2025 at 6:16 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Palo Alto, California was in a collision involving a passenger car on [XXX] near [XXX]. The Waymo AV was parked at the curb facing northwest on [XXX] when a passenger car traveling northwestbound on [XXX] proceeded to parallel park at the curb directly in front of the Waymo AV. While the passenger car was maneuvering to park, the passenger car reversed and the rear of the passenger car made contact with the front of the parked Waymo AV. On December [XXX], 2025 at 10:02 AM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a heavy truck in a parking lot at [XXX]. The Waymo AV was traveling southbound in the parking lot at [XXX] when it slowed to a stop to yield to a heavy truck that was facing southwest and executing a reversing turn into a loading dock parking stall at the east side of the parking lot. The heavy truck came to a stop, then began to proceed forward in the southbound direction, and the Waymo AV proceeded forward again, following the heavy truck. The heavy truck then came to a stop and began to reverse. The Waymo AV then momentarily reversed before coming to a stop and the heavy truck continued to reverse, and the rear right side of the heavy truck trailer made contact with the front left side of the stationary Waymo AV. A pickup truck that was initially stopped directly behind the Waymo AV proceeded forward, and the front of the pickup truck made contact with the rear of the stationary Waymo AV. On December [XXX], 2025 at 5:11 PM, an AVRIDE autonomous vehicle (the "AVRIDE AV") was in a collision in Dallas, Texas on [XXX] near [XXX]. The AVRIDE AV was traveling southeastbound on [XXX] engaged in autonomous mode when it approached a pickup truck parked on the right curb with both doors open. As the AVRIDE AV proceeded to pass the parked pickup truck, the right side of the AVRIDE AV made contact with the open front left side door of the parked pickup truck. Following the collision, the AVRIDE AV pulled over to the side of the road. Both vehicles sustained minor damage. There were no reported injuries. There was no indication of injury or request for medical attention at the scene. Avride received notification of injury from the other party's insurance company on January 16, 2026. Because of this, Avride made an update in the report changing the highest severity injury alleged as "Minor W/O Hospitalization". After following up with the other party's insurance claim, no information or documentation for the alleged injury was provided. Communication from the other party and their representation has stopped as of May 8, 2026. Because of this, we are updating the report again and changing the highest injury severity alleged to "Property Damage. No Injury Reported."

On December [XXX], 2025 at 10:54 PM ET a Waymo Autonomous Vehicle ("Waymo AV") operating in Atlanta, Georgia was in a collision involving a pickup truck on [XXX] near [XXX]. The Waymo AV was traveling northbound on [XXX] when a pickup truck traveling northbound directly behind the Waymo AV proceeded to change lanes to the left, entering the center two-way left turn lane. As the pickup truck began passing to the left of the Waymo AV, the pickup truck maneuvered right and crossed back into the Waymo AV's lane of travel, and the front right side of the pickup truck made contact with the rear left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. The May Mobility vehicle (Ego) is stopped at a red light, then the Ego begins to accelerate and turn right when the traffic light changes to green. As the Ego accelerates, a pedestrian begins moving and enters the crosswalk from the other side of the intersection. This activates the rule-based crosswalk logic. Ego determines that it is able to stop before the crosswalk without exceeding our max deceleration threshold, which triggers an in-policy stopping reaction for the crosswalk. The Ego brakes to yield for the pedestrian. The vehicle trailing the Ego accelerates, making contact with the rear-end of the Ego. In the 2.5 seconds between the start of the Ego braking and the collision, the rear vehicle accelerated from 0.2 to 6.1 m/s (0.4 to 13.6 mph) without slowing until after the collision occurred. In response to this incident, immediate mitigations were made, in that the vehicle operators (AVOs) operate all intersections in manual mode. This remained in place until January 12, when we received a solution which instituted a reduction of the 3.5 m/s2 parameter, in order to prevent the Ego from yielding to pedestrians in crosswalks that are still far from the Egoâs lane if such yielding would induce braking.

On December [XXX], 2025 at 10:20 AM CT a Waymo Autonomous Vehicle ("Waymo AV") operating in Austin, Texas was in a collision involving a passenger car on [XXX] at [XXX]. As the Waymo AV approached the intersection with [XXX] to turn right, it slowed to a stop to yield to a pedestrian that was crossing [XXX] from the south curb within the marked crosswalk. While the Waymo AV remained stopped, a passenger car traveling northeastbound in the right lane on [XXX] approached the Waymo AV from behind and the front of the passenger car made contact with the rear of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On December [XXX], 2025 at 2:14 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving a first responder vehicle on [XXX] at [XXX]. The Waymo AV was stopped facing south in the second from the right lane on [XXX] for a red light at the intersection with [XXX]. While the Waymo AV was stopped, an active fire truck with sirens activated traveling westbound on [XXX] proceeded to turn right to travel north in the rightmost southbound lane of [XXX], to the right of the Waymo AV. The Waymo AV began to move a short distance to the left to create additional room for the fire truck to pass to the AV's right before coming to a stop to yield to the oncoming fire truck. While the Waymo AV was stopped, the fire truck continued to complete the right turn and the right side of the fire truck made contact with the rear right side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. On December [XXX], 2025 at 1:17 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving a van on [XXX] near [XXX]. The Waymo AV was stopped facing west on [XXX] for a red light at the intersection with [XXX] with its right turn signal activated. After remaining stopped for approximately six minutes a short distance in advance of the stop line at the red light, the Waymo AV passenger opened the rear left side door and exited the vehicle, and a van that was initially stopped directly behind the Waymo AV maneuvered to the left before slowing to a stop behind the Waymo AV. The Waymo AV passenger briefly interacted with the van operator through the van window, and then re-entered the Waymo AV. The van then proceeded to pass the Waymo AV on the left and the right side of the van made contact with the rear left side of the stationary Waymo AV. On December [XXX], 2025 at 1:21 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a first responder vehicle on [XXX] at [XXX]. The Waymo AV was stopped facing east in the left lane on [XXX] for a red light at the intersection with [XXX]. A fire engine with lights and siren activated traveling northbound in the leftmost lane on [XXX] proceeded straight through the intersection with [XXX] before stopping, then reversing into the intersection. The fire engine then proceeded to turn left onto [XXX]. The Waymo AV moved slightly to the right within its lane, then came to a stop to yield to the oncoming fire engine. While the Waymo AV was stopped, the fire engine continued to complete the left turn and the left side of the fire engine made contact with the rear left side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. While the Waymo was traveling through the intersection, the Waymo AV began slowing in the protected region adjacent to a traffic island for a red light displayed at the entrance to [XXX]. While the Waymo AV was slowing, a passenger car that was traveling northbound on [XXX] directly behind the Waymo AV approached the Waymo AV from behind, and the front of the passenger car made contact with the rear of the Waymo AV. The Waymo AV was stopped facing north on [XXX] for a flashing red light at the intersection with [XXX]. The Waymo AV proceeded forward and began shifting to the left as it entered the intersection in preparation for a merge into the single northbound lane of [XXX] on the far side of the intersection. At the same time, a cable car traveling northbound on [XXX] in the left-adjacent cable car trackway proceeded to pass the Waymo AV on the left, and the right side of the cable car made contact with the front left side of the Waymo AV. After the light turned green, the Waymo AV proceeded to make a left turn onto [XXX]. As the Waymo AV was completing the left turn, an SUV that was also turning left in the adjacent leftmost designated left turn lane crossed the dashed white lane line, into the Waymo AV's lane of travel, and the front right side of the SUV made contact with the rear left side of the Waymo AV.
