On September [XXX], 2025 at 11:48 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a motorcycle on [XXX] at [XXX]. The Waymo AV was traveling northbound on [XXX] in the rightmost lane with its right turn signal activated when it slowed to a stop for a queue of traffic for a red light at the intersection with [XXX]. As the light turned green and the Waymo AV began to proceed forward, a motorcycle that was traveling northbound on [XXX] in the second to the right lane changed lanes into the rightmost lane and crossed behind the Waymo AV towards the shoulder, at which point the front of the motorcycle made contact with the rear right side of the Waymo AV. The motorcycle then made contact with the cement traffic barrier on the right side of the roadway. The motorcyclist then fell from the motorcycle and the motorcycle continued without a rider, crossing [XXX] and rolling over the curb and across the sidewalk before coming to a stop on its side on the dirt pedestrian pathway between [XXX] and [XXX]. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. The driver of the motorcycle alleged minor injuries. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. On September [XXX], 2025 at 7:58 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a van on [XXX] at [XXX]. The Waymo AV was traveling northeastbound on [XXX] when it came to a stop for a passenger drop-off near the intersection with [XXX]. While the Waymo AV was parked with its hazard lights activated, a van approached the Waymo AV from behind and entered the oncoming lane of [XXX] to maneuver around the Waymo AV. A passenger vehicle came into view while reversing from a perpendicular parking aisle on the right, which was marked for entry only, and was moving toward the Tesla ADS in the wrong direction against directional road markings. The Tesla ADS reduced vehicle speed and steered to the left. The reversing passenger vehicle continued reversing and made contact with the right side of the Tesla vehicle. Safety monitor was present and no passengers were inside the vehicle.

On September [XXX], 2025 at 10:29 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving a passenger car on [XXX] at [XXX]. The Waymo AV was stopped facing west on Franklin Avenue in the second to the left designated left turn lane for a red light at the intersection at [XXX] Avenue. While the Waymo AV was stopped, a passenger car that was traveling northbound in the rightmost lane on Highland Avenue attempted to make a right turn onto eastbound [XXX] Avenue. As the passenger car was making the right turn, the passenger car crossed the double yellow lane line and the front of the passenger car made contact with the left side of a second passenger car that was stopped facing west in the leftmost designated left turn lane on [XXX] Avenue. The right side of the second passenger car then made contact with the left side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV, the first passenger car, and the second passenger car sustained damage. Waymo later received notice of an alleged minor injury from the passenger in the Waymo AV. A Zoox vehicle operating in autonomous mode was traveling in the right lane on southbound [XXX] and preparing to turn right onto [XXX] in Miami when it slowed to a stop in the crosswalk to properly yield to a left-turning vehicle in the oncoming lane. At that time, an e-scooterist westbound on [XXX] entered the crosswalk against the light at [XXX], then swerved and brushed the rear corner of the stopped Zoox vehicle. The e-scooter and the rider remained upright and continued onto the sidewalk along [XXX]. The Zoox vehicle operator then disengaged from autonomous mode and completed the turn shortly after. The Zoox vehicle operator checked on the e-scooterist, who appeared to have stopped to pick up something they dropped. Law enforcement and EMS were called as the e-scooterist reported injury. The EMS personnel provided the e-scooterist with an ice pack and then left the scene. Neither the Zoox vehicle nor the e-scooter sustained any damage.

On September [XXX], 2025 at 8:16 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving an SUV on [XXX] at [XXX]. The Waymo AV was traveling westbound in the middle of three lanes on [XXX] and came to a stop for a red light at the intersection with [XXX]. An SUV was stopped facing west in the left lane with its hazard lights activated. While the Waymo AV remained stopped, passengers entering the stationary SUV opened the rear right side door. The traffic light then turned green and the Waymo AV remained stopped for the passenger of the SUV standing to the left of the Waymo AV. Once the passengers entered the SUV, the Waymo AV proceeded forward, and the opened rear right door made contact with the rear left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. At approximately 12:03 PM CST on September [XXX], 2025, an Aurora truck with an Aurora ride observer seated inside was traveling southbound on [XXX] in autonomy in Spring, Texas when it struck a mattress box spring in its travel lane. As a preceding light-duty vehicle suddenly slowed and abruptly nudged in its lane, a mattress box spring was revealed on the roadway. As designed, the Aurora truck slowed but due to surrounding actors and travel speed, it elected to drive over the object. After the impact, the Aurora truck pulled to the shoulder in autonomy. Once stopped, the Aurora ride observer inspected the Aurora truck for damage and reported to internal dispatch that the air dam on the front bumper of the vehicle was damaged. The Aurora ride observer then drove the truck in manual to Aurora's terminal in Houston. No injuries were reported.

An unoccupied Zoox autonomous vehicle was operating at a reduced speed in lane #5 on westbound [XXX] towards Las Vegas Boulevard in Las Vegas, with a sedan (Vehicle 2) directly behind. The Zoox vehicle gradually slowed to a stop when it was rear ended by Vehicle 2. Vehicle 2 driver initially exited his vehicle to inspect both vehicles, but did not remain at the scene to exchange information. The Zoox vehicle resumed operation and cleared the scene. Shortly afterward, the Zoox vehicle slowed to a stop due to a low tire pressure fault and was subsequently towed. The low tire pressure was attributed to the crash. The Zoox vehicle sustained minor damage. There were no reported injuries, and the police were not notified.

On September[XXX], 2025 at 9:55 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was stopped facing southwest on [XXX] in a queue of traffic for a red light at the intersection at 4th Street[XXX]. The SUV stopped in front of the Waymo AV proceeded forward a short distance to close the distance with the vehicle in front of it, which was stopped at the red light. The Waymo AV followed by proceeding forward a short distance before both vehicles came to a stop. While the Waymo AV was proceeding forward, the SUV behind the Waymo AV proceeded forward as well, but did not stop when the Waymo AV came to a stop and the front of the SUV made contact with the rear of the stationary Waymo AV. The Tesla ADS was proceeding straight in the center lane of the three lane street. As the Tesla ADS was approaching a green light at an intersection, a dog perpendicular to the Tesla vehicle is seen running into the intersection from the right. The Tesla ADS reduced vehicle speed and steered to the left. The dog proceeded to run onto the main road and toward the Tesla ADS's lane and path. The Tesla ADS continued to reduce vehicle speed and biased left in lane. The dog ran toward the Tesla and made contact with the bottom of the Tesla ADS's front right bumper which caused the dog to be pushed to the right, into the lane and path of an approaching van. As the Tesla ADS continued to proceed forward, the dog appeared behind the van and was seen running away from the street, only to turn around and run back toward the street. Safety monitor was present and no passengers were inside the vehicle.

On September [XXX], 2025 at 2:45 AM CT a Waymo Autonomous Vehicle ("Waymo AV") operating in Austin, Texas was in a collision involving a domestic animal on [XXX] near [XXX]. The Waymo AV was traveling northwestbound on a narrow portion of [XXX] with vehicles parked at the curb in both directions when a domestic animal emerged from in front of a parked vehicle and entered the Waymo AV's lane of travel. The front right tire of the Waymo AV made contact with the domestic animal. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The domestic animal sustained injuries. Waymo is reporting this crash under Request No. 2 of Standing General Order 2021-01. On September [XXX], 2025 at 8:01 AM ET a Waymo Autonomous Vehicle ("Waymo AV") operating in Atlanta, Georgia was in a collision involving a pickup truck on [XXX] at [XXX]. The Waymo AV was traveling westbound in the left lane on [XXX]. A pickup truck that was traveling eastbound in the left turn lane on [XXX] initiated a U-turn onto westbound [XXX] at the intersection with [XXX]. As the pickup truck was completing its U-turn, it stopped facing northwest in the right lane on [XXX], unable to complete the turn due to the north side sidewalk. As the Waymo AV continued through the intersection with [XXX] and began to passed the pickup truck, the pickup truck began to reverse into the Waymo AV's lane of travel and the rear left side of the pickup truck made contact with the front right side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On September [XXX], 2025 at 2:56 PM ET a Waymo Autonomous Vehicle ("Waymo AV") operating in Atlanta, Georgia was in a collision involving a cyclist on [XXX] near [XXX]. A Waymo AV was traveling north on [XXX] near the intersection with the [XXX] when it approached the [XXX] shared use bicycle/pedestrian trail. As the AV approached the crossing with the trail, a cyclist traveling east on the trail approached the crossing with the roadway and continued into the roadway without stopping at the stop sign controlling bicycle traffic from the trail. The bicycle then made contact with the rear driver side of the Waymo AV and the impact caused the cyclist to fall from their bicycle. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV and the bicycle sustained damage.
