On July [XXX], 2025 at 11:39 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Tempe, Arizona was in a collision involving a passenger car on [XXX] near [XXX]. The Waymo AV was traveling southbound on [XXX] and slowing to a stop in response to a passenger car that was traveling westbound on [XXX] and turning right onto northbound [XXX]. As the passenger car was completing its turn, it crossed the center line into the Waymo AV's lane of travel and the driver side of the passenger car made contact with the driver side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. The May autonomous vehicle was travelling in an autonomous vehicle (AV) only lane, on [XXX], approaching the intersection at [XXX] A vehicle travelling to the left of the May shuttle in the non-AV lane attempted to perform a right hand turn across the May vehicles path and collided with the May AV. The primary factor that led to this collision was human error on the part of the other vehicle that was required to yield to through traffic in the AV lane but did not. Right turns across a straight lane are uncommon, and drivers are typically not used to them, however there is a metal street sign that instructs other traffic to yield to the vehicles in the AV lane. There were no autonomy faults or errors which led to this incident. Additionally, there is a second traffic light at this intersection that warns public drivers to yield to the AV shuttle. This light was not operating as it should during this incident. We are working with our partner to maintain the operation of this light and have implemented a mitigation policy of daily inspections of this light and implementing an AVO in service if this light is not functioning.

On [XXX] at 10:31 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a scooterist on [XXX] at [XXX]. The Waymo AV was stopped at the curb facing north on [XXX] for a passenger drop-off when the passenger in the Waymo AV opened the rear right door. As the rear right door was being opened by the passenger, a scooterist that was traveling northbound in the rightmost lane on [XXX] proceeded to pass to the right of the Waymo AV. The left handlebar of the scooter made contact with the open rear right door of the stationary Waymo AV, and the scooterist remained standing after the impact. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The scooterist alleged a minor injury and that the scooter sustained damage. The passenger in the Waymo AV was not belted at the time of the collision, as the passenger was preparing to exit the Waymo AV. On July [XXX], 2025 at 1:27 PM MT two Waymo Autonomous Vehicles ("Waymo AV") operating in Phoenix, Arizona were involved in a collision in the West Parking Lot off of [XXX] at the [XXX]. The Waymo AV was parked facing north in a parking stall in the West Parking Lot off of [XXX]. The Waymo AV proceeded to reverse to exit the parking stall, and came to a stop, preparing to maneuver forward and to the left. While the Waymo AV was stopped, a second Waymo AV ([XXX]) that was parked in the left adjacent parking stall proceeded to reverse to exit its parking stall. While the second Waymo AV was reversing, the initial Waymo AV proceeded forward and the front left of the Waymo AV made contact with the right side of the second Waymo AV. A Zoox vehicle (Vehicle 1) operating in autonomous mode was traveling south on [XXX] in the right lane and slowing for a red emergency traffic signal at [XXX] in Las Vegas. Shortly after the Zoox vehicle came to a stop, a pickup (Vehicle 2) following directly behind was rear ended by an SUV (Vehicle 3) that failed to stop. Vehicle 2 then rear ended the stopped Zoox vehicle. The police arrived at the scene. The driver of Vehicle 3 reported that their "head hurt" but declined medical attention. The Zoox vehicle sustained minor damage.

On July [XXX], 2025 at 1:57 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Santa Clara, California was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was stopped on [XXX], preparing to turn right onto northeastbound [XXX] while yielding to oncoming traffic. Once oncoming traffic had cleared, the Waymo AV began to turn right into the rightmost lane on northeastbound [XXX]. As the Waymo AV was completing the right turn, the test driver transitioned the Waymo AV to manual mode. The test driver changed lanes without a signal activated, crossing through the center travel lane and into the dedicated left turn lane where the test driver began to make an unprotected left across southwestbound [XXX]. During this unprotected left turn, an SUV was traveling in the leftmost southwestbound lane. The SUV began to maneuver towards the rightmost lane while the test driver was completing the left turn, and the front of the SUV made contact with the passenger side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was not engaged and a test driver was operating the Waymo AV in manual mode. Both vehicles sustained damage. On July [XXX], 2025 at 1:35 AM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a passenger car on [XXX] near the [XXX] entrance. The Waymo AV was traveling northeastbound on [XXX] in the right lane. A passenger car was traveling southwestbound on the wrong side of median-divided [XXX], directly in the Waymo AV's lane of travel. The Waymo AV braked and initially maneuvered left to avoid the oncoming passenger car. However, the passenger car continued to proceed, maneuvering towards the Waymo AV. The Waymo AV then continued to brake and maneuvered back to the right, to again attempt to avoid the passenger car, but the passenger car once more maneuvered towards the Waymo AV. As the Waymo AV continued to brake and the passenger car continued off the southeast side of [XXX], the front passenger side of the passenger car made contact with the front passenger side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. On July [XXX], 2025 at 8:28 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving an SUV on [XXX] at [XXX]. The Waymo AV was stopped in the rightmost lane facing eastbound on [XXX] at a red traffic light at the intersection with [XXX]. An SUV traveling southbound on [XXX] made a left turn onto [XXX]. As the SUV approached the Waymo AV, the SUV continued turning and moving forward, turning slightly into the center lane and the front passenger side of the SUV made contact with the rear driver side corner of the stationary Waymo AV, and the front driver side of the SUV made contact with the rear passenger side corner of a passenger car that was stopped in the center lane. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both the Waymo AV and the SUV sustained damage, and Waymo cannot confirm the presence or absence of damage to the passenger car as of the date of this submission. A Zoox vehicle (Vehicle 1) in autonomous mode was traveling in the left lane on southwest-bound [XXX] toward [XXX] in San Francisco when a ball, immediately followed by a dog, entered the roadway from between parked cars on the left side of [XXX] directly into the path of the Zoox vehicle. The Zoox vehicle braked in response, but its underbody made contact with the dog. The Zoox vehicle operator then disengaged autonomous mode and brought the vehicle to a stop. The Zoox vehicle operator then reversed the vehicle to free the dog. The dog owner did not indicate injuries to the dog at the scene. The dog and its owner then returned to the sidewalk and the Zoox vehicle operator drove the vehicle from the scene. The police were not called. Zoox subsequently received an email alleging injuries to the dog, "abrasions behind its left front leg and a possible cracked rib."

Summary: This updated report does not report a new incident or make any material changes to the factual record. It only removes confidential or personally identifying information to make the incident narrative publicly available. The Tesla ADS was stopped on the right side of the street. Safety monitor requested support to assist with the Tesla ADS not proceeding forward. The teleoperator took over vehicle control and gradually increased vehicle speed and turned the Tesla ADS left toward the left side of the street. The Tesla vehicle was driven up the curb and made contact with a metal fence. The safety monitor was present and no passengers were inside the vehicle. On 03/20/2026, while submitting this report and removing confidential or personally identifying information, the report was submitted in error indicating airbag deployment and tow for the subject vehicle. This is in error and the reports were updated to reflect that no airbags were deployed and no tow of the Tesla vehicle was conducted.

On Wednesday, July [XXX], a Nuro vehicle operating in autonomous mode stopped at sign-controlled intersection, then began to cautiously proceed through the intersection to make a left turn. Just after the Nuro vehicle began to move into the intersection, it was struck from behind by another vehicle.

A Zoox vehicle (Vehicle 1) operating in autonomous mode was traveling south on [XXX] and initiating a left turn from a dedicated signalized turn lane onto [XXX] in Las Vegas with a flashing yellow. The Zoox vehicle operator disengaged just prior to the signal turning from flashing yellow to green arrow as they continued to execute the left turn. At that time, a passenger car (Vehicle 2) traveling north on [XXX] failed to stop for the red traffic signal and entered the intersection at a high speed, colliding with the passenger side of the Zoox vehicle. The side impact caused the Zoox vehicle to roll onto its driver's side. Both vehicles sustained major damage with airbag deployments. The police and EMS arrived at the scene. The Zoox vehicle operator was transported to a medical facility by an ambulance, and Vehicle 2's driver was reported to have declined medical attention. Both vehicles were towed from the scene.

While traveling north on [XXX], the Autonomous Vehicle Operator (AVO) engages autonomy 3.0 m before the planned right turn and 7.7 m before the break in the curb while traveling at 8.0 m/s. The May Autonomous Vehicle (ego) was planning to make a tight right turn into a parking lot, which the AVO was unaware of when engaging autonomy. The ego jerked to the right upon entering autonomy, hitting a curb and damaging both passenger-side front and rear tires and the right rear tire rim. The AV attempted the right turn despite not having time to slow sufficiently and mounted the curb. The AVO intervened by accelerating and steering back to the left upon striking the curb, manually maneuvering back to the street while striking a second curb on the opposite side of the parking lot driveway. Resimulations without the manual intervention show the ego slowing appropriately and making the right turn into the parking lot. The AVO should have been attentive to the planned route before engaging autonomy. As a result of this incident, planned improvements to the AVOs instructive dash and audio alerts for upcoming turns have begun to be implemented.

On June [XXX], 2025 at 10:04 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a cyclist on [XXX] at [XXX]. The Waymo AV was stopped facing southeast in the right lane on [XXX] for a red light at the intersection at [XXX] when a Waymo AV passenger opted to end their ride prior to reaching their originally specified drop-off location, prompting the Waymo AV to park in-lane and activate its hazard lights for the passenger drop-off. A cyclist was traveling southeastbound on [XXX] in between vehicles in the right lane and vehicles stopped at the curb. As the cyclist passed the Waymo AV to the right, a Waymo AV passenger opened the front passenger side door and the front passenger side door of the Waymo AV made contact with the cyclist. The passengers in the vehicle were not belted at the time of the collision, as they were preparing to exit the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. The cyclist reported minor injuries and was transported from the scene in an ambulance. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because an individual involved was transported from the scene to a hospital for medical treatment and a vehicle involved was towed away. On June [XXX], 2025 at 11:35 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Scottsdale, Arizona was in a collision involving an unattended bicycle in the roadway on [XXX] near [XXX]. The Waymo AV was traveling eastbound in the rightmost lane of [XXX] when the Waymo AV's front and undercarriage made contact with an unattended bicycle that was laying in the middle of the roadway. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage. One of the passengers in the Waymo AV was not belted at the time of the collision. Waymo is reporting this crash under Request No. 2 of Standing General Order 2021-01. Waymo may supplement or correct its reporting with additional information as it may become available.

On June [XXX], 2025 at 11:03 PM ET a Waymo Autonomous Vehicle ("Waymo AV") operating in Atlanta, Georgia was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was parked at the curb facing north on [XXX] when an SUV reversed out of a driveway on the west side of [XXX], and the rear of the SUV made contact with the rear driver side of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained damage.
