An unoccupied Zoox autonomous vehicle (Vehicle 1) was traveling northeast on a green traffic light on [XXX] at [XXX] in San Francisco. At that time, a box truck (Vehicle 2), heading southwest on [XXX], suddenly attempted an unprotected left turn onto [XXX]. The Zoox vehicle braked hard to yield to the rapidly approaching Vehicle 2, when the left side mirror assembly of Vehicle 2 made contact with the roof-mounted left rear sensor pod of the Zoox vehicle. The Zoox vehicle stopped after the contact, and the Vehicle 2 operator completed the turn onto [XXX] and pulled over. Shortly afterward, the Zoox vehicle cleared the intersection, pulled over, and then returned to base. The Zoox vehicle sustained sensor damage, and no injuries were reported.

On September [XXX], 2025 at 6:59 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, California was in a collision involving an SUV on [XXX] at [XXX]. The Waymo AV was stopped facing northwest on [XXX] for a red light at the intersection at [XXX], while an SUV was stopped facing southeast on [XXX] with its left turn signal activated. When the light turned green, the Waymo AV proceeded forward into the intersection. The SUV then began an unprotected left turn onto northeastbound [XXX], and the front left of the SUV made contact with the left side of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Summary: This updated report does not report a new incident or make any material changes to the factual record. It only removes confidential or personally identifying information to make the incident narrative publicly available. The Tesla ADS was stopped in the bi-directional turn lane (or center left turn lane) of a five lane road with its left indicator active. When the two lanes of oncoming traffic were clear, the ADS made an unprotected left turn into an entrance of a parking lot. As the ADS entered the parking lot, the ADS made contact with a metal chain. The ADS had a safety monitor present with one passenger in the vehicle. On 03/20/2026, while submitting this report and removing confidential or personally identifying information, the report was submitted in error indicating airbag deployment and tow for the subject vehicle. This is in error and the reports were updated to reflect that no airbags were deployed and no tow of the Tesla vehicle was conducted.

On August [XXX], 2025 at 12:42 PM MT a Waymo Autonomous Vehicle ("Waymo AV") operating in Phoenix, Arizona was in a collision involving a section of removed pavement on [XXX] near [XXX]. The Waymo AV was traveling southbound through a construction zone on [XXX] in the rightmost lane delineated by a line of temporary orange pylons when it approached a parked construction truck in its lane of travel with a mounted arrow board displaying a flashing left arrow. The Waymo AV changed lanes to continue traveling southbound on [XXX] to the left of the line of pylons. As the Waymo AV continued to follow the path delineated by the pylons, it encountered a section of the pavement that had been removed creating a drop in the lane surface, and the front left wheel proceeded over the section of removed pavement.. On August [XXX], 2025 at 1:50 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a motorcycle on [XXX] at [XXX]. On August 16, 2025 at 7:47 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a heavy truck on [XXX] and [XXX]. On August [XXX], 2025, one of Nuro's Prius vehicles operating in manual mode in the Houston area was struck from behind while yielding to cross-traffic at an intersection. The vehicle had been operating in autonomous mode within 30 seconds prior to the incident, but the operator had disengaged autonomy several seconds prior to reaching the intersection. No injuries resulted from this incident, although there was minor property damage to both vehicles.

At approximately 10:55 AM CST on August [XXX], 2025, an Aurora truck was traveling westbound on [XXX] in autonomy during a development mission with an Aurora vehicle operator near Stephenville, Texas. The Aurora truck was in the right lane and approaching an unmarked right-lane closure with no advance warning signs. The Aurora vehicle operator decided to disengage autonomy and attempted to manually merge the Aurora truck into the left lane. During the vehicle operators manual lane change, a passing tractor trailer sped up on the left and made contact with the Aurora truck. The Aurora vehicle operator attempted to alert the other tractor trailer of the impact to the Aurora truck by honking the horn and flashing lights. However, the passing tractor trailer proceeded without stopping, and after being unable to gain the other drivers attention, the Aurora vehicle operator pulled over to the shoulder of the highway to assess damage to the Aurora truck. The Aurora vehicle operator reported damage to the trucks left side mirror and sensor equipment to Auroras dispatch before returning to Auroras terminal in Aledo, Texas. No injuries were reported.

An unoccupied Zoox vehicle in autonomy was traveling north on [XXX] toward [XXX] in San Francisco and slowing for an SUV (Vehicle 2) ahead, also heading north on [XXX]. As the Zoox vehicle approached the nearly stopped Vehicle 2, it steered left to pass. The driver of Vehicle 2 then, without using a turn signal, abruptly began reversing in an apparent attempt to parallel park on the right side of [XXX]. The Zoox vehicle then braked to a near stop when it was struck on the right rear wheel by Vehicle 2 as it swung into a parking space. Shortly afterward, the Zoox vehicle was towed from the scene for inspection. The Zoox vehicle did not sustain damage, but the left front bumper of the SUV appeared to have minor damage. There were no reported injuries, and the police were not called.

An unoccupied Zoox autonomous vehicle was traveling west along [XXX] toward [XXX] in Las Vegas when a tractor-trailer (Vehicle 2) operator failed to yield the right-of-way, failed to stop at a stop sign, and quickly pulled out of a driveway crossing the centerline and entering the Zoox vehicle's travel lane in an apparent attempt to make a left turn onto westbound [XXX]. The Zoox vehicle braked hard and steered right within its travel lane, but contact was unavoidable. The right corner of the front bumper and grille of the tractor-trailer made contact with the left front side of the Zoox vehicle. The Zoox vehicle then came to a complete stop. The operator of Vehicle 2 reversed off the roadway and stayed on-site to exchange information with the Zoox operations team. The Zoox vehicle was towed from the scene for inspection and was found to have sustained moderate damage. The front bumper, hood, and grille on Vehicle 2 appeared to be damaged. There were no reported injuries, and the police were not called.

On August [XXX], 2025 at 4:54 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in San Francisco, California was in a collision involving a passenger car on [XXX] at [XXX]. The Waymo AV was stopped in the rightmost lane facing northeast on [XXX] in a queue of traffic at the intersection with [XXX] at a location where the rightmost lane and leftmost lane are separated by a raised transit boarding island. As the queue of traffic advanced, the Waymo AV began moving forward. A scooterist traveling northeastbound in the rightmost lane proceeded to pass to the right of the Waymo AV followed closely by a cyclist, and the Waymo AV shifted left towards the raised boarding island and slowed to a stop to yield to the scooterist and cyclist. A passenger car traveling behind the Waymo AV approached the Waymo AV from behind and the front of the passenger car made contact with the rear of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. The passenger of the Waymo AV later alleged a minor injury. The May vehicle was travelling in an autonomous vehicle (AV) only lane approaching an intersection. A vehicle travelling to the left of the May shuttle (ego) in the non-AV lane performed a right hand turn across the May vehicle¢ÃÂÃÂs path. Given the full stopping power and the limited space with which the ego had to stop, the ego was physically unable to stop in time before impact. The primary factor that led to this collision was human error on the part of the other vehicle that was required to yield to through traffic in the AV lane but did not. Right turns across a straight lane are uncommon, and drivers are typically not used to them, however there are several signs that instruct other traffic to yield to the vehicles in the AV lane. There were no autonomy faults or errors which led to this incident. Update: There was no indication of injury or request for medical attention at the scene. The police report states "Driver 1 complained of neck pain." For this reason, we are updating the highest severity injury alleged field to reflect the âminor w/o hospitalizationâ injury level. Before the Waymo AV could initiate the left turn, the Waymo AV slowed to a stop within the left lane to yield to a scooterist that was traveling southwestbound on the sidewalk across the entrance of the parking garage. While the Waymo AV was stopped, a passenger car approached the Waymo AV from behind and the front of the passenger car made contact with the rear of the Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. Waymo is reporting this crash under Request No. 2 of Standing General Order 2021-01. Waymo may supplement or correct its reporting with additional information as it may become available.

An unoccupied Zoox autonomous vehicle was stopped at a green traffic light on eastbound [XXX], waiting for traffic ahead to clear to turn left onto [XXX] in San Francisco. A pickup truck (Vehicle 2) was also stopped for the same traffic light, directly behind the Zoox vehicle. After the light turned yellow, the Vehicle 2 driver passed the stopped Zoox vehicle on the left side, striking the left rear fender, and proceeded through the red light. The Vehicle 2 driver did not stop to exchange information. Shortly afterward, the Zoox vehicle turned left onto [XXX] to clear the scene, briefly stopped again, and then pulled over. The Zoox vehicle was then towed for inspection. It sustained minor damage. There were no reported injuries and the police were not called.

On August [XXX], 2025 at 10:15 AM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Santa Monica, California was in a collision involving a passenger car on [XXX] at [XXX]. The Waymo AV was traveling southeastbound in the left lane on [XXX] when it began to change lanes to the right lane and came to a stop for the red light at the intersection with [XXX] and to yield to law enforcement and emergency vehicles that were blocking vehicles from continuing southeastbound on [XXX]. While the Waymo AV remained stopped partially in the left lane and partially in the bike lane, a law enforcement officer arrived and began to direct traffic around the Waymo AV. A passenger car that was initially stopped behind the Waymo AV proceeded to maneuver around the left of the Waymo AV, and the right side of the passenger car made contact with the rear left of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The passenger car sustained damage. While the Waymo AV was maneuvering around the passenger car on the right, a passenger in the passenger car opened the rear right side door and the door made contact with the left side of the Waymo AV. On August [XXX], 2025 at 4:41 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Miami, Florida was in a collision involving the exterior wall of a gas station store at [XXX]. The Waymo AV was traveling westbound in autonomous mode on [XXX] when the Waymo AV test driver transitioned to manual mode near [XXX]. The test driver continued westbound on [XXX] and made a right turn at the intersection with [XXX] and then a left turn into the [XXX] at [XXX]. The test driver traveled westbound within the gas station and then made a right turn towards a parking stall in front of the gas station store. As the test driver was about to complete the parking maneuver, they accelerated forward towards the exterior wall of the gas station store and the front of the Waymo AV made contact with the cement bumper and pillars in front of the gas station store and the exterior wall of the store. At the time of the impact, the Waymo AV's Level 4 ADS was not engaged and a test driver was operating the Waymo AV in manual mode. The Waymo AV, the pillar, and the exterior wall sustained damage. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away.
